Liability Waiver

I understand that there is always risk when dogs are in attendance even when the best precautions are taken to prevent such incidents.
I acknowledge that my dog/s is current on all vaccinations required for their age against viral and communicable disease. A current copy of vaccination records must be seen prior to start of class/ daycare or the start of. Any dog that arrives without such records must not participate until records have been seen.
I hereby waive and release, The Dog’s Den Learning Center Ltd., it’s owner, employees, agents and clients, from any and all liability of any nature, for injury or damage which I, attending family members, possessions, guest or my dog may suffer while in attendance of any training session, daycare or other function of the center or while on the training grounds or surrounding area thereto. I hereby agree to indemnify and hold harmless, The Dog’s Den Learning Center Ltd., it’s owner, employees, agents and clients from any and all claims, or claims by any member of my family, or any other person accompanying me to any training session, daycare or any other function offered by the center or while on the grounds or surrounding area thereto as a result of any action by any dog, including my own or for any other reason. I expressly assume the risk of damage, or injury for my participation in training sessions, daycare or other function of, The Dog’s Den Learning Center Ltd. or while on the training grounds or surrounding area.
In case of illness or injury while attending daycare every effort will be made to contact owner. Dogs needing immediate medical attention will be taken to a vet of Daycares choosing. All costs incurred will be the full responsibility of owner for payment to vet. Only basic necessary treatment will be authorized by daycare. All other treatment must be authorized by owner. The Dog’s Den Learning Center Ltd.  is not responsible for any payment to vet for medical treatment. Owner is responsible for all expenses incurred during holding period for unclaimed dogs. After written notice to owners address and 10 days, The Dog’s Den Learning Center Ltd. takes legal ownership of said dog and will re-home to place of their choosing.
 There will be no refund 24 hours before the start of the first class of single courses, obedience bundles and pre-paid daycare packages. Obedience bundles are purchased as one unit that includes three or four courses and daycare. Daycare packages and specials are purchased at a significant discount from the daily rate. Before purchasing, be sure that you want all of what you are purchasing. A service charge will apply for cancellations done 48 hrs prior to start of scheduled class/pre-paid daycare, in the amount of 30.00.

I agree to all of the above and that this contract is in effect beginning on below date and remain in effect on all future dates that said dog attends training/daycare/related functions at The Dog’s Den Learning Center Ltd.
