Dog Adoption and Pre-screening Form

All dogs adopted through The Dogs Den Adoption Service are required to attend at least one training course with The Dogs Den. The cost of this course is included in the adoption fee of $300.00. The remaining funds are transferred to a local shelter of your choice to help other animals in need.

There are multiple requests for the same dog. The completion of this form allows us to choose homes best suited to the dog being placed for adoption. Those homes that are considered suitable will be contacted to arrange a time to meet the dog. If you are not chosen for the dog you are applying for we would be happy to keep your file for a period of 2 months and contact you when we receive a match. Please let us know if you would like to be put on a waiting list.
